Request for Late Enrollment

This form is to be used only to request a late enrollment during the Request for Late Enrollment Period listed on the Important Dates page. After submitting this form it will be routed for department approval. Students should submit this form as early as possible so departments have a chance to review and approve it. If the form does not have department approval 24 hours after the deadline to submit the form, then the enrollment cannot be processed. If there is a hold on your record the enrollment will not be processed.

Please note that enrollment after the add/drop deadline may result in changes to a student’s financial aid award. A $25 late registration fee will be assessed for students who currently have no enrollment.
