Withdrawal Form
Before you withdraw. If you need assistance with counseling, finances, academics or any other issue visit the website for resources. Withdrawing from classes will impact your time to graduation and may have financial aid implications.    

Deadlines for Withdrawals.  Deadline dates are listed on the Important Dates page. Upon submission, your form will be routed to your instructor for approval and then to Registrar’s for processing. Your instructor must approve the form by the end of the business day after the withdrawal deadline in order for the Office of the Registrar to review/process the request. You may track the status of the form and approvals in MyCompass.

Freshmen Withdrawal forms for freshmen will be routed to the appropriate FASST (freshmen) advisor for approval prior to being routed to the instructor. International student (F1/J1) forms will be routed to CIE for approval of withdrawal.

Medical withdrawals After submitting this online form an email will be sent to your horizon email. You must open that email and attach medical documentation on letterhead to complete submission of the withdrawal form or your withdrawal request will not be reviewed/processed.

NOTE: You need to fill out one withdrawal form for each class that you are withdrawing from.
NOTE: For medical withdrawal, after submitting this form an email will be sent to your horizon email. You must open that email and attach medical documentation on letterhead to complete submission of the withdrawal form or your withdrawal request will not be processed. This medical documentation must clearly demonstrate how the condition prevented you from completing the course(s) and you should provide a clear and detailed explanation when listing your reason for the withdrawal in the box below. If approved by the Office of the Registrar, the units will not count toward the 18 semester unit limit.
